마취 환자의 스트레스 완화에 미치는 음악 효과 연구: 심박변화도를 중심으로

초록

본 연구의 목적은 심박변화도(HRV)를 스트레스 관련 생리학적 변화에 대한 비침습적 도구로 활용하여 마취 상태로 수술받는 동안의 환자가 경험하는 스트레스를 줄이는데 음악의 어떠한 영향을 미치는가를 알아보는 것이다. HRV는 자율신경계의 교감 및 부교감 활동과 관련이 있다. 본 연구에서 HRV 데이터는 전신 마취(n=5) 또는 척추 마취(n=5) 하에서 복부 수술(n=5) 또는 정형외과 골절 수술(n=5)을 받은 환자 10명으로부터 수집하였다. 마취가 된 시간동안 환자들에게 이완음악(Relaxation music)을 들려주고, 고주파(HF)와 저주파(LF)의 합으로 계측된 HRV의 고주파 비율을 분석하였다. 음악 감상전에 비해 음악감상 이후에 HF의 평균이 증가했으며, 이는 통계적으로 유의미한 (P=0.035) 결과를 나타냈다. 음악감상전 HF 평균에 비해, 전신 마취±음악일 때 HF 평균이 약간 증가했다는 점에 주목했다(P=0.063). 따라서, 본 연구결과 수술하는 동안의 음악감상은 HF 데이터 평균 증가를 유도하여 부교감신경계를 활성화시키고, 교감신경계를 비활성화하여 이완을 유도함을 확인하였다.
