초록

관상동맥 혈관조영 과정에서 환자들은 높은 수준의 스트레스와 불안감을 경험한다. 본 연구는 진정제 섭취와 고통, 불안, 만족 수준 등에 모바일 어플리케이션을 통한 환자 통제 음악감상 중재법의 효과 조사를 위한 비무작위 통제로 시행되었다. 국소 마취와 진정제를 투여하고 관상동맥조영을 하기로 예정된 환자 76명을 모집하였다. 이중 54명은 음악실험집단에 22명은 통제집단에 배정되었다. 통증 점수, APAIS, 통증 만족 점수를 이용한 불안수준을 측정하였으며, 음악 중재 과정 동안의 섭취한 투약 내용도 기록되었다. 중재 과정에서 음악감상을 경험한 연구참여자들들은 상당히 적은 미다졸람 처방을 받았다(중간 값1 vs 통제군 3mg). 또한 불안수준에서 유의미한 차이가 나타났으며, 통증 점수에서는 유의미한 결과는 관찰되지 않았다. 음악감상 집단에 속한 거의 모든 참여자들은 만족감을 보고하였으며, 통제집단의 환자중  91%가 만족감을 보고하였다. Music-Care 프로그램 어플리케이션은 국소마취 및 진정제를 투여하고 관상동맥 조영술을 하는 동안 미다졸람 진정제 투여를 2/3로 감소하도록 유도하였다. 심장병 전문의들은 본 연구결과를 높이 평가하였으며, 외래진료에서 유용한 도움이 될 것이라 제안하였다.
